But how about the price of a new hip? The cost of medical procedures like hip replacements can vary by thousands of dollars \u2013 even tens of thousands of dollars.<\/em><\/p>\n<p><em>We talked with Sean Cavanaugh, deputy director of the federal government\u2019s Center of Medicare, about who\u2019s paying what and why.<\/em><\/p>\n<hr \/>\n<h4>Breaking down the costs<\/h4>\n<p><em>Hospital bills are high \u2013 but Medicare patients don\u2019t foot the entire bill. Actually, a lot is left unpaid.<\/em><\/p>\n<p><strong>Question: <\/strong>Two years ago the average hip replacement in Texas cost about $48,000 \u2013 that was the average hospital charge. Medicare only paid about $9,000 of that and patients were responsible for $1,300. What do you make of that discrepancy?<\/p>\n<p><strong>Sean Cavanaugh: <\/strong>&#8220;Hospital charges are often very disconnected from what Medicare and other payers are actually paying hospitals. Credit: College of American Pathologists\/Flickr<\/p><\/div>\n<h4>After the hip replacement<\/h4>\n<p><em>The sticker price for a new hip doesn\u2019t tell the whole story. Much of the cost &#8212; and what makes the bill so difficult to predict &#8212; comes after a patient leaves the hospital.<\/em><\/p>\n<p><strong>Question: <\/strong>Is there any way to determine how much a Medicare beneficiary can expect to pay for something like a hip replacement?<\/p>\n<p><strong>Cavanaugh:<\/strong> &#8220;It\u2019s complicated. A patient can probably get a pretty good sense of what they\u2019ll owe for the hospital portion of their stay, what the hospital likely will be paid. And they also know in Medicare benefits what their hospital deductible is, which is around $1,200. Credit: Shutterstock<\/p><\/div>\n<p data-atavist-id=\"at53a0565fdc5a8\">&#8220;Where it gets more complicated is that patient will also be seen by physicians and he or she doesn\u2019t know how often those physician visits will occur, but the patient will be liable for the deductible and the cost sharing associated with physician visits.<\/p>\n<p>&#8220;Where it grows increasingly complex is the course of treatment the patient receives after they\u2019re discharged from the hospital. A hip fracture is a perfectly good example. A patient with a hip fracture could be discharged to home but more typically these patients would be discharged to an inpatient rehabilitation facility. They could be discharged to a skilled nursing facility. Medicare only paid about $9,000 of that and patients were responsible for $1,300.<\/p><\/div>\n<p><strong>Question<\/strong>: In <a href=\"http:\/\/www.washingtonpost.com\/blogs\/wonkblog\/wp\/2013\/02\/12\/how-much-does-hip-surgery-cost-somewhere-between-10000-and-125000\/\" target=\"_blank\">one study looking at the cost of a new hip<\/a>, prices were all over the map, from $10,000 to $125,000, and data shows <a href=\"http:\/\/archinte.jamanetwork.com\/article.aspx?articleid=1569848\" target=\"_blank\">Medicare will often pay between $10,000 and $25,000 for primary joint replacement surgery<\/a>. Is this inconsistency of cost a concern for Medicare?<\/p>\n<p><strong>Cavanaugh<\/strong>: &#8220;Yes. It interests us greatly. The reason those payments vary so much is not because of how much Medicare pays the individual providers. It\u2019s because of how many services are used in different regions and the types of services. This is particularly true around where the patient is discharged after they leave the hospital, whether they go to another rehab facility, go home and what therapies they receive. The reason we\u2019re so interested and concerned in this area is there are regions and providers who have very expensive post-hospitalization expenses, but they are not associated with higher-quality care or better results. We feel this is an opportunity for the industry to learn from each other, to learn from the lower-cost areas of how to reduce costs while still improving quality care that the beneficiary receives.&#8221;<\/p>\n<h4>Seeking better care<\/h4>\n<p><em>Medicare is trying to be more transparent with who\u2019s paying what in the hopes of encouraging better care.<\/em><br \/>\n<strong>Question<\/strong>: The cost of hip replacements doesn\u2019t say anything about quality. We have a website called Hospital Compare where patients can see voluminous data of the quality of care that hospitals are providing. That\u2019s information that\u2019s available to the beneficiary. We\u2019re also doing things with providers. We\u2019re increasingly linking how much they get paid to the quality of care they provide.
